A Candidate for admission is required to provide photocopies of the documents at the time of admission.

• Kindly put 'N/A' in the Team/Individual section of the form. 

Required Documents

1. A printout of the form with one photo pasted in a square-shaded box.

2. Date of Birth Certificate from Nagar Nigam/ Gram Panchayat OR  Aadhar Card (One Photocopy).
